Tacrolimus met mycofenolaatmofetil en prednison (off-label. Tacrolimus + azathioprine + prednison of tacrolimus + everolimus + prednison.

Possible equal value

Op dit moment is de combinatie van tacrolimus met mycofenolaatmofetil (en prednison) al de standaardbehandeling (waarbij MMF off-label wordt gegeven). Een alternatief is de combinatie tacrolimus + azathioprine + prednison, of tacrolimus + everolimus + prednison. Voor de eerste combinatie is dat inferieur aan de combinatie met MMF (aangezien er meer rejecties zijn), voor de combi met everolimus is dat therapeutisch gelijkwaardig. Bij niertransplantaties is de tacrolimus + MMF + prednison het meest gangbaar en dat zal vermoedelijk nog vele jaren zo blijven.

In 2023 stonden in Nederland 69 patiënten tussen de 0 en de 15 jaar op de wachtlijst voor een hart- nier- of levertransplantatie (1). Het gaat jaarlijks om beperkte aantallen. Voor een harttransplantatie komen er jaarlijks ongeveer 5 kinderen per jaar in aanmerking en bij levertransplantatie ongeveer 20 kinderen per jaar (2).
