Substantiation Het grootste deel van de patiënten met chronisch handeczeem kan goed behandeld worden met lokale corticosteroïden. Als dit dagelijks nodig is met hoge klasse kan delgocitinib ingezet worden als corticosteroïd-sparend geneesmiddel. Het is ook zinvol als er bijwerkingen zijn van lokale corticosteroïden zoals atrofie van de huid. De verwachting is dat delgoctinib geïndiceerd zal worden bij ernstig handeczeem, na falen op topicale steroïden. Het zal dan concurreren met bijvoorbeeld alitretinoine. Uit de recent gepubliceerde 2b studie wordt niet duidelijk wat de effectiviteit is van delgocitinib bij de verschillende maten van ernst van handeczeem. Om daar antwoord op te krijgen moeten we de resultaten van delta 2 en 3 afwachten.

Additional remarks Een Nederlandse cohort studie in de algemene bevolking liet zien dat 1,9% van de mensen boven de 25 jaar ooit ernstig handeczeem heeft gehad (1). Het geneesmiddel alitretenoïne, dat in dezelfde lijn ingezet wordt had ongeveer 4.000 gebruikers in 2021. Er wordt verwacht dat deze patiëntengroep mogelijk ook delgocitinib kan gaan gebruiken.

Additional remarks In een Japanse kosteneffectiviteitsanalyse wordt uitgegaan van het volgende: 'drug cost for delgocitinib per 4 weeks was calculated to be 21,739 JPY'. Dit betekent dat delgocitnib omgerekend €136,87 per patiënt per 4 weken zal kosten. Dit komt neer op €1.642 per patiënt per jaar. De prijs in Nederland is echter nog niet bekend.
