Substantiation JAK-remmers worden veelal ingezet na falen van anti-TNF bij IBD. Reeds langer ervaring met deze klasse bij UC, lijken minder effectief bij CD (hogere NNT). Trials met upadacitinib laten een positief resultaat zien. Mogelijk van waarde voor een deel van de CD-patiënten. Dit zullen veelal falers op de diverse biologicals zijn.

Additional remarks Er zijn momenteel in Nederland ruim 90.000 mensen met een chronische darmontsteking (1). Hiervan heeft iets minder dan de helft de ziekte van Crohn (2). In 2021 werd ustekinumab aan iets minder dan 2.000 patiënten met Crohn voorgeschreven en vedolizumab aan bijna 1500 patiënten (3). Upadacitnib zal naar verwachting met name ingezet worden bij patiënten waar behandeling met biologicals niet aanslaat. Uit onderzoek blijkt dat na 52 weken behandeling het aantal falers op beide producten zo'n 49% bedraagt (4,5). Het maximale aantal patiënten wordt daarom ingeschat op 1541.
