Additional remarks Tofacitinib is een krachtige, selectieve remmer van de JAK-familie. In humane cellen remt tofacitinib preferentieel signaaltransductie door heterodimere cytokinereceptoren die JAK3 en/of JAK1 binden, met een functionele selectieve voorkeur boven cytokinereceptoren die signalen doorgeven via paren van JAK2. Toedieningsvorm: wordt aangeboden als tablet en drank.

Substantiation De fase 3 studie toonde duidelijke effectiviteit aan ten opzichte van placebo. Daarnaast zal tofacitinib mogelijk eerder ingezet worden dan toculizumab in het geval van prikangst bij kinderen. Daarnaast is tofacitinib ook werkzaam tegen het ontstaan van uveitis bij patiënten met Juveniele Idiopathische Artritis.

Additional remarks Juvenile Idiopathic arthritis komt voor bij ongeveer 4.000 kinderen in Nederland met ongeveer 30-40 nieuwe gevallen per jaar. Het patiëntvolume zal 300 tot 500 patiënten bedragen.

Additional remarks AIP (exclusief BTW) Xeljanz 5mg 56 stuks: €694,22. Prijs per 5mg tablet = €12,40. Aangezien de behandelduur chronisch is en de aanbevolen dosering 2x daags 5mg is voor patiënten zwaarder dan 40kg zullen de kosten per patiënt per jaar maximaal €9.052 (730 tabletten*€12,40) bedragen. Er is nog geen indicatie voor de lijstprijs van de oral solution formulering. De lijstprijs wordt (mede) vastgesteld op basis van de therapeutische waarde ten opzichte van andere beschikbare therapieën. Hierbij zal ook rekening worden gehouden met de prijzen van huidige beschikbare behandelopties. De lijstprijs van de oral solution formulering zal niet voor de 2e helft van 2021 bekend zijn.
